IMPORTANT:  When completing the order form please provide the initials of the player at the bottom of the form so it can be printed onto the kit.  Please also make sure you enter the appropriate size for each item ordered.  We use Nike apparel for our training kit so if in doubt please check sizes at a local store that sells Nike clothes before completing your order form.
